Looking for a Helping Hand in Ohio?


Life can be tough sometimes, and everyone needs a little support now and then. Here
at the Knox County Community Health Center, we’re committed to helping our community,
regardless of their situation. One of the resources we recommend is the Ohio Benefits
website. It’s filled with great information to help you and your family make important
decisions about important benefits.


How can Ohio Benefits provide you and your family with support?


The Ohio Benefits website offers a variety of options. You'll start by creating a secure
profile and answering some quick Self-Assessment questions. These questions are not
applications themselves, but they help you see if a specific program might be a good fit
for you and your family. If the assessments indicate eligibility, you'll be guided through
the application process.


Benefit Options Available:
 Check Eligibility for Cash, Food, or Medical Assistance: See if you qualify for
low or no cost health care, help paying Medicare premiums, SNAP (food
assistance or food stamps), or cash assistance.
 Apply for Cash, Food, Medical, or Child Care Assistance: This involves
creating a Log in and creating an account
 Check Eligibility for Early Childhood Services: Explore options for your
children like Early Childhood Education Grant, Early Head Start, Head Start, Help
Me Grow Early Intervention, Help Me Grow Home Visiting, Preschool Special
Education, or Publicly Funded Child Care.
 Access My Cash, Food, Medical, or Child Care Assistance: This involves
creating a Log in and creating an account.


Additional Support and Resources:
 Stay Informed: Sign up for project reminders and benefits updates through text
messages and/or voice calls. There are many options and ways they can guide
you and notify you. And don’t we all love reminders and help?
 Veteran Benefits: If you’re a Veteran and served in the S.S. military, you may be
eligible for veteran benefits. The website has a form to complete to explore these
options. This also applies to spouse/widow(er) benefits.
 Extensive Resource Section: The website features a comprehensive section with
answers to frequently asked questions, information about additional programs
and services, and valuable support resources. Medicaid information is available
as well.

We encourage you to take a look at the Ohio Benefits website if you are in need of

provide you with support and guidance, as well.

We are dedicated to providing comprehensive healthcare services for children of all ages. Our goal is to help your child thrive, both physically and emotionally. We offer a wide range of services, from routine checkups and immunizations to dental care and mental health counseling.

More Than Just a Walk-In Clinic:

We are your primary care provider, offering preventive care and personalized attention. We treat patients of all ages and backgrounds, regardless of their insurance status. We accept Medicaid and most major insurance plans, and we offer a sliding fee scale for those who qualify.

Complete Care for Your Child:

Dental:

Our Dental Clinics are available at Knox County Community Health Center and Knox County Dental. Did you know we have a full-time dentist, oral surgeon, dental assistants, and dental hygienists available for you and your children?

We offer routine dental care and more.

  • Fillings
  • Sealants
  • X-rays
  • Cleanings
  • Extractions (including wisdom teeth)
  • Child Dentistry

 

Payment Policy

  • Payment is due at time of service.
  • Minimum payment for self-pay patients is $20 for Dental.
  • If patients do not have health insurance, a sliding fee schedule at 200% above federal poverty guidelines is offered to all patients who are eligible. Need help getting coverage? VisitHealthCare.govfor easy steps to get health insurance. 
  • Health Center staff can also assist patients with enrolling in health insurance or Medicaid if they qualify.
  • We accept Cigna Dental, Delta Dental, CareSource, Molina and All Ohio Medicaid Plans.
  • Patients are expected to pay the full sliding-fee-scale amount at the time of their appointment.
  • At the time your appointment is made, you will be told how much you can expect to pay. If for some reason we need to do a procedure that was not planned, and you cannot pay in full, a statement will be given to you. The remaining balance will need to be paid before another appointment can be scheduled.
  • KCCHC protects its patients from surprise medical bills for services and items provided by KCCHC through a combination of: compliance with requirements established under Section 330 of the Public Health Service Act and provision of Good Faith Estimates as outlined in Section 27966B-6 of the Federal No Surprises Act.

Medical:

Our medical clinics at Knox County Community Health Center, Mount Vernon Health Center, Danville Community Health Center, & our Walk-In Clinic are all available for your child’s medical needs.

We offer:

  • Treatment of illnesses, such as earaches, colds, sore throats, infections, and more
  • Physical exams for school and sports
  • We-child exams
  • Lead testing
  • Immunizations for children including influenza, hepatitis, DTaP, MMR, Tdap, HPV, RSV, Rotavirus, IPV, and more
  • Tetanus shots
  • Nutrition counseling
  • Substance abuse treatment

Payment Policy

  • Payment is due at time of service.
  • Minimum payment for self-pay patients is $20 for Medical.

Mental Health:

Counseling provides all individuals with an opportunity to work on emotional healing. With the support of a counselor, your child can work through whatever difficulties are affecting their lives. Counseling is available at Knox County Community Health Center, Mount Vernon Health Center, and Danville Community Health Center.

We offer:

  • Therapy for Depression, Anxiety, PTSD, Anger Management, Grief, and more
  • Individual counseling for all ages
  • Play therapy for children
  • Diagnostic assessments
  • Substance Use Disorder counseling for adolescents
  • Case management

Payment Policy

  • Payment is due at time of service.
  • A sliding fee schedule is offered to all patients.
  • Minimum payment for self-pay patients is $10 for counseling services.
  • If patients do not have health insurance, a sliding fee schedule at 200% above federal poverty guidelines is offered to all patients who are eligible. Need help getting coverage? VisitHealthCare.govfor easy steps to get health insurance. 
  • Health Center staff can also assist patients with enrolling in health insurance or Medicaid if they qualify.
  • We accept all Ohio Medicaid Plans, private insurance plans, and self-pay.

We also offer the Children with Medical Handicaps program. We don’t want you to feel alone if you have a child with medical handicaps. We know that it is a full-time job caring for your child and you may feel that you have no one in your corner. We offer the Children with Medical Handicaps Program (CMH) through Knox Public Health as part of our lineup of services to support you and your child. Read more about this program here.

The Mount Vernon Health Center is Now Open!

Are you getting excited? We are! Our newest Health Center—the Mount Vernon Health Center is NOW OPEN! Our Knox County Community Health Center options continue to grow, allowing us to serve our community better. Don’t worry—this means the Moore Family Practice is just moving one block away from their current location into the new Mount Vernon Health Center. The current Moore Family Practice building will then close, and all providers will occupy the new building.

General Information

When will the new building open?

We officially opened Monday, January 29, 2024.

What are the hours of operation?

We are open Monday through Friday from 8:00am-4:30pm.

Do you have parking? Do you have to pay? Is it on a bus route?

Parking is free. The Knox Area Transit can be used for travel to the center. The Health Center can pay for expenses if the patient cannot afford it.

Services

What existing services will be provided?

All existing services (Medical services and Behavioral Health) from Moore Family Practice will be provided.

Are there new services at this new location?

We will not offer new services at this time. We are hoping to add immunizations in the future, so stay tuned!

Can I walk in and be seen without an appointment? Do you offer same day services?

We do have a limited number of same-day appointments available. If those appointments are full, we do have a Walk-in Clinic available just one block away for any acute needs. But we always welcome walk-ins. We are here for YOU!

Do I get to choose my doctor or practitioner?

Yes, we do everything in our power to schedule your appointment with the provider of your choice; however, all our providers provide the same care to all of our patients.

Do you have online access to my records via a patient portal?

Yes, via the Healow App. When you become a patient of our Health Center you can enroll in our Patient Portal via your email. It is easy and convenient for appointment reminders, after visit summaries, lab results, and to contact a provider.

Insurance

How do I enroll my insurance?

A patient can fill out insurance information on our Electronic Intake Paperwork link on our website and bring it to the appointment or complete a paper form prior to an appointment.

Will you accept Insurance? Medicaid? Medicare?

All payors are accepted. We are Knox county’s preferred Medicaid provider, and we accept private insurance, Medicaid, and Medicare.

We can’t wait to see you at the Mount Vernon Health Center. Our plan is to have an Open House and ribbon cutting ceremony on March 28th, with more info to come. We are inviting everyone to come see the new place and inform you of what we do and how to utilize our services. The Knox Community Hospital

  • The Knox Community Hospital, located at 1330 Coshocton Ave, Mt. Vernon, Oh regularly
    conducts blood drives. Check their website or community pages for when these are
    held.

American Red Cross

  • The American Red Cross, located at 812 Coshocton Ave Bldg., Unit #4 Mt. Vernon, Oh
    hosts blood drives and events at their office and around the community. Check their
    website for information.

Local Community Centers

  •  Keep an eye on mobile blood drives, community centers, schools, colleges, and churches
    in Knox County, as they often collaborate with blood donation organizations to host
    blood drives.
