Who needs what? (required immunizations are underlined)

• Kindergarteners (4-6 years old) 
  • DTaP - 5th dose
  • IPV - 4th dose
  • VAR - 2nd dose
  • MMR - 2nd dose
• Middle Schoolers (11-12 years old) 
  • Tdap
  • HPV - Did you know... if you start the series before 15 years old, only two vaccination are required. After 15 yeras old, three vaccinations are needed to complete the series.
  • Meningococcal - 1st dose
• High Schoolers (16 years old) 
  • Meningococcal - 2nd dose
  • Meningitis B (required by some colleges)

 

Physicals available during normal business hours at our Mount Vernon location and at the Danville Community Health Center

• Sports Physicals - $20 (not submitted to insurance)
• Wellness Exam - FREE with most insurance plans; Sliding fee scale for Self Pay
